Job Summary / Description:
Outreach members to complete health assessments, build service plans, care plans and answer member questions about health plan navigation.
Describe the performance expectations/metrics for this individual and their team: Complete 5 to 6 visits per week with all documentation completed Tell me about what their first day looks like: After corporate training, shadow a field staff What previous job titles or background work will in this role?
CCU Care Coordinator, DRS Counselor
Pay: $18
– $22 hourly (plus mileage reimbursement once travel is required / pay determined on experience)
Shift: Monday-Friday 8am-5pm
Required Skills:
2+ years of Experience in a healthcare / managed care organization Aging/ DRS / Waiver experience is preferred Bachelor’s Degree in healthcare related field or equivalent experience Well versed with computer & typing skills; Experience documenting in a clinical system Preferred Skills:
Medicare / Medicaid experience preferred but not a must have Master’s Degree in Social Work, Nursing, or similar Background in Medical Management, in a managed care environment preferred.
